PRÉSENTATION

La douleur chronique affecte une personne sur cinq au Canada. Cette condition peut avoir de lourdes conséquences aux niveaux physique, émotionnel et social. Plusieurs individus consultent une multitude de professionnels de la santé en quête de soulagement, mais la douleur chronique reste difficile à traiter. Comprendre comment la douleur aiguë devient chronique est essentiel au développement de stratégies de prévention et d'intervention précoce afin de réduire les risques de chronicité de la douleur. De plus, il importe de souligner que la douleur chronique peut prendre plusieurs formes. Si l'on souhaite prévenir la douleur chronique et améliorer la gestion de la douleur chronique, il importe donc de comprendre l'hétérogénéité inter et intra individuelle dans l'expérience de la douleur, d'identifier différents sous-groupes d'individus qui partagent des trajectoires similaires de douleur et d'identifier les facteurs de risque pour des trajectoires de douleur non-résolue. Nos projets de recherche combinent une perspective biopsychosociale de la douleur avec des approches méthodologiques innovantes pour comprendre la nature dynamique de l'expérience de la douleur.

Dre Gabrielle Pagé et son équipe reçoivent un financement des IRSC (classés 2e sur 29), dans le cadre du projet printemps 2023 des IRSC pour le projet CircaDouleur. Ce projet vise à découvrir certaines similitudes et différences entre les expériences de douleur individuelles, afin d'optimiser la gestion de la douleur. Le montant de la bourse est de 956,250$.

Douleur au dos et stress
Le stress mène à la production de glucocorticoïdes, ce qui peut avoir un effet paradoxal sur la douleur, incluant une augmentation de la douleur perçue (hyperalgésie) ou une diminution de la douleur perçue (hypoalgésie). Des décennies de recherche ont permis d'identifier quatre caractéristiques de situations qui, lorsqu'elles sont présentes, vont stimuler la réponse physiologique du stress, notamment la perception faible de contrôle, l'imprévisibilité, la nouveauté, et l'égo menacé (CINÉ). Cette étude vise à examiner les associations entre le stress et la douleur parmi les individus vivant avec de la douleur au dos en utilisant le modèle CINÉ. Nous pourrons ainsi mieux comprendre les éléments stressants associés à l'expérience de la douleur et ainsi développer des interventions personnalisées basées sur ces facteurs.
Projet COMMORBID
La douleur chronique est comorbide avec plusieurs maladies physiques et mentales. La gestion des maladies chroniques requière une grande implication du patient dans ses propres soins. Dans un contexte de multicomorbidité, cette gestion de plusieurs conditions médicales peut entraîner plusieurs difficultés dont les difficultés à prioriser les symptômes et les tâches d'autogestion des maladies. Cette étude vise à explorer les mécanismes de priorisation des symptômes dans un contexte d'autogestion complexe.
Trajectoires de douleur
Il y a autant de trajectoires de douleur qu'il y a d'individus vivant avec la douleur. Que l'on parle de douleur chronique ou de douleur post-chirurgicale, il est important de comprendre les trajectoires de douleur ainsi que les facteurs qui prédisent l'appartenance à l'une ou l'autre de ces trajectoires. Différents projets de recherche vise à examiner cette question.
Opioïdes
Tout comme la douleur chronique, l'utilisation d'opioïdes peut devenir chronique. Il n'est pas rare que des individus consomment toujours des opioïdes des mois après une chirurgie. La plupart des études qui examinent l'utilisation prolongée d'opioïdes utilisent des bases de données médico-administratives qui ne contiennent pas de données sur la douleur et les facteurs de risque biopsychosociaux. Certaines de nos études explorent les facteurs de risque d'une utilisation prolongée d'opioïdes. Une autre étude vise également à documenter l'impact d'une prise en charge multidisciplinaire avant et après la chirurgie sur l'incidence de la douleur chronique et l'utilisation prolongée des opioïdes.
REVEAL(OT)
La douleur chronique est l’un des problèmes de santé les plus souvent associés à la présence d’incapacités et de défis dans les
activités de la vie quotidienne et au travail. Peu d’interventions ciblent directement les difficultés de fonctionnement des personnes affectées par la douleur chronique. Les ergothérapeutes possèdent une expertise unique pour documenter et intervenir sur les défis quotidiens associés à la douleur chronique. Ce projet vise à adapter et développer l'intervention Redesign your Everyday Activities and Lifestyle with Occupational Therapy (REVEAL(OT)) dirigée par des ergothérapeutes, au contexte québécois. Cette intervention multidisciplinaire a le potentiel de bonifier l’offre de soins actuelle et mieux répondre aux besoins.
CircaDouleur
La douleur chronique a des impacts sur toutes les dimensions de la vie, pourtant, elle n'en demeure pas moins invisible pour l'entourage. Certaines personnes ont une douleur qui change au cours de la journée, d'autres ont une douleur constante ; la douleur de certaines personnes a un impact sur leur humeur et leur sommeil, d'autres non. Le projet CircaDouleur vise à découvrir certaines similitudes et différences entre les expériences de douleur de chacun. À terme, le but est de pouvoir gérer la douleur de manière plus efficace.
